THE AHSF BRINGS ARIZONA HEALTHCARE PROFESSIONALS TOGETHER TO HELP EMPOWER ARIZONANS TO MAKE THEIR LIFE WISHES AND CARE DIRECTIVES KNOWN AND TO EQUIP THEIR HEALTHCARE TEAMS WITH RESOURCES TO HONOR THESE DECISIONS. DURING 2023: *AZHHA, SUPPORTED BY THE AHSF, TAUGHT CLASSES FOR ADVANCE CARE PLANNING, CONVERSATIONS IN SERIOUS ILLNESS AND POLST TO A TOTAL OF 37 PEOPLE IN 8 CLASSES IN 2023. AHSF SERVED AS THE STATE LEAD FOR POLST. *AZHHA, ON BEHALF OF ARIZONA, WITH THE SUPPORT OF THE AHSP MAINTAINED A MEMBERSHIP IN NATIONAL POLST.*AZHHA MAINTAINED A CONTRACT WITH UNIVERSITY OF CA, SAN FRANCISCO FOR AN ARIZONA VERSION OF PREPARE FOR YOUR CARE, WHICH IS HOUSED ON AZHHA'S WEBSITE.
