TO IMPROVE THE HEALTH AND WELL-BEING OF ALL TEXANS, PARTICULARLY THE UNDERSERVED, BY OPENING DOORS TO HEALTHCARE AND HEALTH-RELATED SERVICES. OUR MISSION IS ACHIEVED THROUGH: 1) COMMUNITY: COLLABORATING WITH COMMUNITY-BASED PROVIDERS AND ORGANIZATIONS TO IMPROVE ACCESS, QUALITY, COORDINATION AND COST-EFFECTIVENESS OF SERVICES. 2) HEALTH: DEVELOPING PROGRAMS TO ESTABLISH MEDICAL HOMES, MANAGE HEALTH CONDITIONS, AND PROMOTE WELLNESS AND PREVENTIVE CARE. 3) CHOICE: ENCOURAGING PERSONAL ACCOUNTABILITY AND EDUCATED CHOICES FOR INDIVIDUAL AND FAMILY HEALTH. FOR INDIVIDUAL AND FAMILY HEALTH.
