THE ALZHEIMER'S COMMUNITY CARE FOUNDATION, INC. (THE FOUNDATION) WAS INCORPORATED IN THE STATE OF FLORIDA AS A NOT-FOR-PROFIT CORPORATION IN JULY 2008. THE FOUNDATION WAS ORGANIZED EXCLUSIVELY FOR THE BENEFIT OF ALZHEIMER'S COMMUNITY CARE, TO BE THE CUSTODIAN OF FINANCIAL GIFTS AND TO PROMOTE AND ADVISE PHILANTHROPIC ACTIONS FOR THE COMMON GOOD OF ALZHEIMER'S COMMUNITY CARE.
